Pharmacy

Using the Hamilton Pharmacy
Patients with prescriptions from Hamilton Community Health Network Physicians need to:

  1. Identify yourself as a patient from Hamilton.
  2. The pharmacist will fill your prescription with medications purchased through pharmacy program.
  3. If your insurance does not cover medications, you will be able to purchase your medication at a greatly reduced rate from the Burton or Clio Pharmacy.
  4. If you have an existing prescription that you would like to transfer, just bring your pill bottle to the Burton or Clio Pharmacy.
  5. Support your Health Center by participating in this program at the Clio Road and Burton Pharmacies.
  6. Hamilton Community Health Network will screen patients for eligibility to use the 340B Discount Pharmacy Program at the Clio Road and Burton Pharmacies.
