Who is Eligible?

Only patients of Hamilton Community Health Network are eligible to use the 340B Discount Pharmacy Program.

When using the HCHN Discount Pharmacy:

Patients with prescriptions from HCHN Physicians need to:

Identify yourself as a patient from HCHN.
The pharmacist will fill your prescription with medications purchased through pharmacy program.
If your insurance does not cover medications, you will be able to purchase your medications at a greatly reduced rate from the Burton or Clio Pharmacy.
If you have an existing prescription that you would like to transfer, just bring your pill bottle to the Burton or Clio Pharmacy.
